Sample Assessment

Instructions for completing the 8-ates™ assessment:

Left click on the 8-ates word (Communicate, Delegate, Educate, etc) in the left cell of the table below for a pop-up window that will provide additional information on that behavior.
Rate the individual on a scale of 1 to 4 by left clicking on the appropriate button for each behavior.

      1 -- Indicates the person needs significant improvement. His/her behaviors in this area are having considerable negative impact on the effectiveness of the team/department/organization.
      2 -- Indicates the person is "OK" in this area, but could be better. His/her behaviors in the area may not be having significant negative impact, but there is not significant positive impact either.

      3 -- The person being rated does this pretty well; i.e., better than most of his/her peers. The behaviors in this area are for the most part having positive impact on the team, but could be even better.

      4 -- The individual is exceptional in this area - significantly better than any of his/her peers.

Half-point increments are also available (1.5, 2.5, and 3.5)
Type any suggestions you might have as to what you would like to see the individual Start, Stop, and/or Continue doing in each of the 8-ates™ to improve his/her effectiveness as a leader/manager. (You can "copy/paste" the text from the pop-up windows as a starting point for your suggestions in each text box). One feedback entry is required for each of the 8-ates™ i.e., you must enter something in at least one of the Start, Stop, or Continue text boxes for EACH of the 8-ates™. Please be as specific as possible with your comments, so the individual you are rating will be able to understand what behaviors you would like to see more of, and/or less of, or how you would like him/her to behave differently.
